Guidelines for Nomination
- Complete the appropriate nomination form below*.
- Provide a brief (500 words or less) description of the nominee's achievement(s) (nature, scope and impact).
- Attach any letters of reference in support of this nomination and include the names of the references on the Awards Nomination Form. Letters of reference are not a mandatory requirement but may contribute positively to the nomination.
- Attach a current Curriculum Vitae for the nominee.
